
Jennifer Hans, PharmD, BCPS
Residency Program Director
Dr. Jennifer Hans is a graduate of the University of Florida College of Pharmacy. She graduated in 2012 and has served as a: Clinical Pharmacist at Northside Hospital in St. Petersburg, Florida and Oak Hill Hospital in Brooksville, Florida; Pharmacist in Charge at PharmaLabs, LLC, in St. Petersburg, FL; Compounding Pharmacist at Trinity Medical Group in Trinity, FL, specializing in USP 795; and The Compounding Shop in St. Petersburg, FL, specializing in USP 795 and 797. Dr. Hans has been the Pharmacy Clinical Coordinator at Oak Hill Hospital since May of 2017 and prior to that, she was a clinical pharmacist (2015). Dr. Hans became a Board Certified Pharmacist in Pharmacotherapy in October 2018 and maintains a license to administer immunizations.
She is also the Pharmacy Residency Program Coordinator at Oak Hill Hospital and helped build the new PGY1 Pharmacy Residency Program. Her passion for patient care inspired her to advance the clinical pharmacy program at Oak Hill Hospital in order to ensure the team provides superior clinical pharmacy services and excellent patient satisfaction. In her spare time, Dr. Hans enjoys spending time with her husband and step-children, traveling, Burn Boot Camp and theme parks.

Gabriella Sarver, PharmD, BCPS
Residency Program Coordinator
Gabriella Sarver, PharmD, BCPS is a Pharmacy Clinical Specialist at Oak Hill Hospital. Born and raised in the Tampa Bay Area, Dr. Sarver graduated from the University of South Florida with a BS in Biology, later receiving her Doctorate of Pharmacy from the USF Taneja College of Pharmacy. She then completed an ASHP-accredited PGY-1 residency at AdventHealth Tampa in Tampa, FL. Dr. Sarver began working at Oak Hill Hospital shortly after completing her residency as a Clinical Pharmacist, after which she transitioned to the role of Pharmacy Clinical Specialist and obtained her Board Certification in Pharmacotherapy. In her free time, she enjoys cooking, baking, playing with her two cats and crocheting.

Lori Brown, PharmD, BCPS, CPh
Director of Pharmacy
Dr. Lori Brown is an accomplished Director of Pharmacy for Healthtrust at Oak Hill Hospital. She received her PharmD from the University of Minnesota, Twin Cities and completed a residency at the University of Minnesota Medical Center, Fairview. She is Board Certified in Pharmacotherapy, and has authored several publications in the area of pain pathways and opioid tolerance research. She relocated to Florida in 2006, and has held positions as Clinical Coordinator of Pharmacy at Northside Hospital, Clinical Manager of Pharmacy for Cardinal Health at Town and Country Hospital, and Director of Pharmacy at Tampa Community Hospital. She has served as the president for the Southwest Florida Society of Health System Pharmacists (SWFSHP), and has been the recipient of the Florida Society of Health Systems Pharmacists (FSHP) Pharmacist of the Year Forerunner Award" in 2012 which recognizes a pharmacist in the State of Florida who emerged early in their career as a leader, and has contributed significantly to the advancement of the profession of pharmacy.
She enjoys mentoring future leaders of pharmacy and has been recognized as a LECOM School of Pharmacy "Preceptor of the Year" in 2014. She is an active member of the community, as a recent member of the Tampa Bay Waterski Show Team, and a certified yoga instructor, often teaching classes to healthcare employees to raise donations for the American Heart Association. In her spare time, Dr. Brown enjoys yoga, mountain biking and camping.

Tracy Augugliaro, PharmD
Clinical Pharmacist
Tracy Augugliaro, RN, BSN, PharmD is a clinical pharmacist at Oakhill Hospital. Dr. Augugliaro graduated with her Doctorate of Pharmacy Degree from the University of South Florida in 2017. She came to Oakhill Hospital in February of 2019. In addition she has 18 years of experience as an ICU nurse, the last 5 of those years were spent in the Cardiovascular ICU. Other experiences include almost 2 years of retail experience. She is married with 2 girls. She enjoys family time and scrapbooking.

Andrew Carothers, PharmD, BCPS
Clinical Pharmacist
Dr. Andrew Carothers has been a clinical pharmacist at Oak Hill Hospital for more than three years. Dr. Carothers graduated with his PharmD from the University of South Florida in 2016. While Dr. Carothers describes Oak Hill Hospital as a busy facility due to the high volumes, he feels the people he works with (both in and outside of the pharmacy) more than make up for any hardship due to the atmosphere of community they have all worked hard to build. Dr. Carothers says he wouldn't trade his position for any other pharmacy job, and encourages candidates to come see why for themselves. He is currently studying to take the BCPS exam, expecting to receive his certification in spring 2020. Some of his interests include physical fitness, hiking, and playing video games.

Mike Putze, PharmD, CPh
Clinical Pharmacist
Michael graduated from Duquesne University with a Pharmacy Doctorate and completed the Pharmacy Management Certificate Program at Duquesne University. Michael has served in various leadership positions in the Tampa Bay area, including Hospital Director of Pharmacy positions and President of the Florida Society of Health-System Pharmacists. Michael has consulted for Hospital Pharmacy Management and Retail Pharmacy for Institutional and Retail pharmacies throughout the United States.
Michael has a passion for pharmacy operations. He has completed Lean Six Sigma Black Belt Training with Cardinal Health. In his free time, Michael enjoys soccer, running, and trading bitcoin.

Amy Golding, PharmD
Clinical Pharmacist
Amy Golding, Pharm.D is a Clinical Pharmacist at Oak Hill Hospital in Spring Hill, Florida. Dr. Golding was raised in Buffalo, NY and is an avid fan of hockey, the Buffalo Sabres, and her son's travel hockey team. Dr. Golding graduated from Massachusetts College of Pharmacy with her Doctor of Pharmacy degree and joined Oak Hill Hospital in 2010. She has been a pharmacist for more than 10 years and worked in various settings including retail for Target Pharmacy. Dr. Golding is a Pharmacy Preceptor for Critical Care at Oak Hill Hospital. Some of her interests include traveling out of state for her son's hockey tournaments, visiting Canada, and running.

Kandy Griswold, RPh
Pharmacy Operations Coordinator
Kandy Griswold, BS, RPh, is originally from Broughton, Illinois. She received her Bachelor of Science Degree of Pharmacy from the St. Louis College of Pharmacy in St. Louis, Missouri in 1989. Her career began at Kare Drugs in the National Supermarket chain in St. Charles, Missouri. After five years of retail, she returned to her Southern Illinois roots and family for a change of career atmosphere into the hospital pharmacy world. She began as a Staff Pharmacist at Harrisburg Medical Center, a 75 bed rural hospital, then transitioned into the Director of Pharmacy for 10 years. She then progressed into a dual role of Consulting Pharmacist / DOP for the OmniCare Company for a short period of time. Preferring hospital over consulting brought her back to a full time role as DOP for Hardin County General Hospital, a 25 bed critical access hospital in Rosiclare, Illinois.
In 2011 she moved to Hernando Beach, Florida and obtained a Clinical Pharmacist position at Oak Hill Hospital in Brooksville, Florida. She has now advanced into her current position as the Pharmacy Operations Coordinator for Oak Hill Hospital. In her spare time, Kandy enjoys boating, camping, kayaking, and participating in a local book club.

Merlyn Jose, PharmD, BCPS
Pharmacy Clinical Specialist
Merlyn Jose, PharmD, BCPS is a Pharmacy Clinical Specialist at Oak Hill Hospital, Brooksville, Florida. Born and raised in Kerala, India, Dr. Jose moved to the United States in 2003. She graduated from Lake Erie College of Osteopathic Medicine in 2014. She has been a pharmacist for more than 5 years and is experienced in both operations and clinical duties. Her interests include infectious diseases and critical care medicine. Prior to joining Oak Hill Hospital, Dr. Jose worked as a Clinical Pharmacist at Northside Hospital and Tampa Bay Heart Institute.
Dr. Jose is an active member of Florida Society of Health- System Pharmacists (FSHP) and American Society of Health-System pharmacists (ASHP). In her free time, she enjoys spending time with family, traveling, and cooking.

Mike Liguori, PharmD, BCPS, CPh

Michael Liguori, BS Pharm, PharmD, C.Ph., BCPS, is a clinical staff pharmacist working primarily in operations. Dr. Liguori previously worked in a variety of Administrative roles serving as Director of Pharmacy at HCA, Bayonet Point as well as systems Director of Pharmacy for both Brooksville and Spring Hill Regional Pharmacy prior to joining the Pharmacy staff at HCA, Oak Hill Hospital in 2004. Dr Liguori earned his Bachelors of Pharmacy Degree at the University of Florida in 1982 as well as his Doctor of Pharmacy Degree in 1998. Dr Liguori is a Board Certified Pharmacotherapy Specialist (BCPS) and has been an active Consultant Pharmacist for over 30 years. Dr Liguori has been a past preceptor for pharmacy student interns through the University of Florida as well as a past preceptor for pharmacist completing requirements for their consultant pharmacist license through the Florida Board of Pharmacy.
Dr. Liguori has been a lifelong resident of Hernando County and has lived in Hernando Beach since 1984. Dr Liguori enjoys boating, fishing, scuba diving and other coastal water sports, in addition to holding a private pilot's license.

Laura Lonergan, PharmD, BCGP

Laura is originally from central New York where she attended the Albany College of Pharmacy. She explored hospital pharmacy before it was fashionable. All of her classmates were eagerly looking in retail pharmacy but she knew she had found her niche in hospital pharmacy. The setting has certainly changed over the years but providing quality pharmaceutical care has remained in the forefront of her practice. The “buzz” words (transitions of care, medication reconciliation) are ever changing but the business of providing a patient safe environment continues on. Further on in her career, decentralizing the pharmacist staff out to the patient care areas became her next career step. To her pleasure the relationship with the nursing staff improved drastically whereby they worked together as a team. Providing medication education to the patients and their families proved to be rewarding. Her other opportunity in pharmacy came as a Long-Term Care Pharmacy Consultant. She came to learn all the special pharmaceutical issues pertaining to the elderly. She enjoyed visiting different nursing homes every month and seeing the residents. It was extremely rewarding tapering residents off unneeded psychotropic medications. She helped institute antibiotic stewardship programs into the nursing homes. Her favorite experience was working with a geriatrician to “DE prescribe” medications in the elderly and discontinuing medicine when the benefit no longer outweighs the risk. DE prescribing is individualized per patient and has a great impact on the resident’s quality of life. Due to her interest in long term care she earned her Board Certification in Geriatric Pharmacy. In April, she came to sunny Florida and ended up at Oak Hill Hospital still enjoying the challenges of Hospital Pharmacy. She states “it’s been fun exploring Brooksville and the surrounding areas. I can’t wait to hit the beach and have friends from NY come and visit”.

Nicholas Micchice, PharmD
Clinical Pharmacist
Nicholas Micciche, PharmD is an Advanced Clinical Pharmacist at Oak Hill Hospital. He received his Doctorate in Pharmacy from the University of South Florida, Taneja College of Pharmacy. After originally starting as a Clinical Pharmacist at Bayfront Health Spring Hill, Nick came to Oak Hill in March of 2020 to begin improving patient safety/outcomes in his role in the Medication Reconciliation program in the Emergency Department. His interests include emergency medicine and oncology. In his free time, he enjoys spending time with his son Lincoln, traveling, and watching the Tampa Bay Buccaneers.

Robert Pietras, PharmD, BCPS, BCCCP
Supervisor Medication Reconciliation
Clinical Pharmacy Specialist
Robert completed his bachelors of science in 2010 in biochemistry at the University of Florida. Throughout which he worked as a senior lab technician at the University of Florida’s Molecular Pathology Core performing special stains and immunohistochemistry staining of various tissue samples. He also supported the quantification of various pancreas samples from the Network for Pancreatic Organ Donors.
Pharmacy school was consummated in 2014 as Summa Cum Laude, while obtaining a residency with Turley Family Medical Center which specialized in ambulatory care. During pharmacy school Robert worked with Medisca Networking to help facilitate sterile and non-sterile compounding courses.
After completion of his residency, he obtained a position as a clinical pharmacist at Regional Medical Center Bayonet Point in the summer of 2015. Despite have an ambulatory care focused education, it soon became evident that critical care was a favored area of practice in an inpatient setting. The main area of work was typically in one of the 3 intensive care units. During employment Robert lead pharmacist attendance at rapid responses and codes. Upon co-proctoring a trauma residency rotation, also helped incorporate pharmacist attendance to trauma alerts. Several lectures were also provided to the medical staff (CME) as well as the residents at the noon conference (GME).
In early 2020, Robert got word that a new pilot program was beginning in the hospital chain at a sister hospital named Oak Hill. This program would place pharmacy technicians and a pharmacist in the ER to obtain medication histories and reconcile them at admission. Robert led the team in implementing this program with the direction of Oak Hill’s pharmacy director Lori Brown and Hospital Corporation of America’s corporate office.

Jean Xavier, PharmD, CPh
Clinical Pharmacist
Jean Xavier was born in Latortue, Haiti. After Dr. Xavier’s third visit to America, he finally immigrated with his green card and has lived in America since. Dr. Xavier applied to Lindsay Hopkins, Miami for his ESL classes. After being admitted he later completed all the required courses and graduated. With his Basic English courses completed, he continued to further his education by moving to Jacksonville and attending college there. There he received his AA degree from FCCJ, now, FSCJ. In addition, he also received his AS degree in Agriculture at The Christian Technical Institution and Agriculture. Subsequently, he attended pharmacy school at the Florida A&M University and graduated Cum Laude with his doctor of pharmacy in 2005. Dr. Xavier worked as a retail pharmacist at Winn Dixie Pharmacy from 2004-2007. Subsequently Dr. Xavier completed his PGY1 pharmacy residency in 2008 at WJB Dorn VA Medical Center in South Carolina. After completing his residency he worked as a clinical pharmacist at Orlando Health in Orlando, FL until 2012. After this he transitioned to Oak Hill Hospital in Brooksville, FL and has been a clinical pharmacist there since 2012. Dr. Xavier enjoys spending time with his wife and 10 month old son.

Peggy Wu, PharmD
Informatics Pharmacist
Peggy Wu is originally from Taiwan, where she worked for the pharmaceutical industry. She joined HCA in 2001, and obtained her PharmD degree from the University of Florida in 2005. Dr. Wu has participated in multiple Meditech projects and implemented CPOE and Pyxis (automated medication dispensing system) programs. In her free time, she enjoys spending time with family, singing in a cappella chorus, and volunteering in an animal shelter.

Lauren Kaveski, PharmD, BCPS, works predominantly as an evening clinical pharmacist in internal medicine at Oak Hill Hospital. Prior to this position, Dr. Kaveski has worked as a clinical staff pharmacist in Long Island, New York for over four years. She is a Board Certified Pharmacotherapy Specialist and has completed a teaching certificate program and a PGY1 residency at a community hospital in Long Island, New York. Dr. Kaveski has earned her PharmD from St. Johns University in Queens, New York. She recently moved to Land O’ Lakes in November 2019 and enjoys cycling and all thing outdoors. She is also quite crafty and spends time crocheting, gardening, painting and completing DIY projects.

After originally starting as a Clinical Pharmacist at Bayfront Health Spring Hill, Nick came to Oak Hill in March of 2020 to begin improving patient safety/outcomes in his role in the Medication Reconciliation program in the Emergency Department. His interests include emergency medicine and oncology. In his free time, he enjoys spending time with his son Lincoln, traveling, and watching the Tampa Bay Buccaneers.

Robert Pietras, PharmD, BCPS

Robert completed his bachelors of science in 2010 in biochemistry at the University of Florida. Throughout which he worked as a senior lab technician at the University of Florida’s Molecular Pathology Core performing special stains and immunohistochemistry staining of various tissue samples. He also supported the quantification of various pancreas samples from the Network for Pancreatic Organ Donors.
Pharmacy school was consummated in 2014 as Summa Cum Laude, while obtaining a residency with Turley Family Medical Center which specialized in ambulatory care. During pharmacy school Robert worked with Medisca Networking to help facilitate sterile and non-sterile compounding courses.
After completion of his residency, he obtained a position as a clinical pharmacist at Regional Medical Center Bayonet Point in the summer of 2015. Despite have an ambulatory care focused education, it soon became evident that critical care was a favored area of practice in an inpatient setting. The main area of work was typically in one of the 3 intensive care units. During employment Robert lead pharmacist attendance at rapid responses and codes. Upon co-proctoring a trauma residency rotation, also helped incorporate pharmacist attendance to trauma alerts. Several lectures were also provided to the medical staff (CME) as well as the residents at the noon conference (GME).
In early 2020, Robert got word that a new pilot program was beginning in the hospital chain at a sister hospital named Oak Hill. This program would place pharmacy technicians and a pharmacist in the ER to obtain medication histories and reconcile them at admission. Robert led the team in implementing this program with the direction of Oak Hill’s pharmacy director Lori Brown and Hospital Corporation of America’s corporate office.

Mike Putze, PharmD, CPh

Michael graduated from Duquesne University with a Pharmacy Doctorate and completed the Pharmacy Management Certificate Program at Duquesne University. Michael has served in various leadership positions in the Tampa Bay area, including Hospital Director of Pharmacy positions and President of the Florida Society of Health-System Pharmacists. Michael has consulted for Hospital Pharmacy Management and Retail Pharmacy for Institutional and Retail pharmacies throughout the United States.
Michael has a passion for pharmacy operations. He has completed Lean Six Sigma Black Belt Training with Cardinal Health. In his free time, Michael enjoys soccer, running, and trading bitcoin.

Gabriella Sarver, PharmD, BCPS
Pharmacy Clinical Specialist
Gabriella Sarver, PharmD, BCPS is a Pharmacy Clinical Specialist at Oak Hill Hospital. Born and raised in the Tampa Bay Area, Dr. Sarver graduated from the University of South Florida with a BS in Biology, later receiving her Doctorate of Pharmacy from the USF Taneja College of Pharmacy. She then completed an ASHP-accredited PGY-1 residency at AdventHealth Tampa in Tampa, FL. Dr. Sarver began working at Oak Hill Hospital shortly after completing her residency as a Clinical Pharmacist, after which she transitioned to the role of Pharmacy Clinical Specialist and obtained her Board Certification in Pharmacotherapy. In her free time, she enjoys cooking, baking, playing with her two cats and crocheting.

Peggy Wu, PharmD
Informatics Pharmacist
Peggy Wu is originally from Taiwan, where she worked for the pharmaceutical industry. She joined HCA in 2001, and obtained her PharmD degree from the University of Florida in 2005. Dr. Wu has participated in multiple Meditech projects and implemented CPOE and Pyxis (automated medication dispensing system) programs. In her free time, she enjoys spending time with family, singing in a cappella chorus, and volunteering in an animal shelter.

Jean Xavier, PharmD, CPh
Clinical Pharmacist
Jean Xavier was born in Latortue, Haiti. After Dr. Xavier’s third visit to America, he finally immigrated with his green card and has lived in America since. Dr. Xavier applied to Lindsay Hopkins, Miami for his ESL classes. After being admitted he later completed all the required courses and graduated. With his Basic English courses completed, he continued to further his education by moving to Jacksonville and attending college there. There he received his AA degree from FCCJ, now, FSCJ. In addition, he also received his AS degree in Agriculture at The Christian Technical Institution and Agriculture. Subsequently, he attended pharmacy school at the Florida A&M University and graduated Cum Laude with his doctor of pharmacy in 2005. Dr. Xavier worked as a retail pharmacist at Winn Dixie Pharmacy from 2004-2007. Subsequently Dr. Xavier completed his PGY1 pharmacy residency in 2008 at WJB Dorn VA Medical Center in South Carolina. After completing his residency he worked as a clinical pharmacist at Orlando Health in Orlando, FL until 2012. After this he transitioned to Oak Hill Hospital in Brooksville, FL and has been a clinical pharmacist there since 2012. Dr. Xavier enjoys spending time with his wife and 10 month old son.
