The Next Presentation in Its Heart Failure Educational Series: “Avoiding Readmission & How Care Assure Will Help”

Spring Hill, FL – The Heart Institute at Oak Hill Hospital announces that it will host its next presentation in its Heart Failure Educational Series on Monday, November 11, 2013 from 4:30 pm – 5:30 pm at H2U Partner’s Club on the northwest corner of the Oak Hill Hospital campus, 11375 Cortez Boulevard, State Road 50, Spring Hill, Florida 34613. The Heart Institute at Oak Hill Hospital will present “Avoiding Readmission & How Care Assure Will Help.” The program will be presented by Lisa Cierpka, RN, BSN, Care Assure Patient Care Navigator.
Nearly four out of ten people who are successfully treated at the hospital for heart failure return within 30 days. The Heart Institute at Oak Hill Hospital wants to change that statistic! Lisa Cierpka, RN, BSN, Care Assure Patient Care Navigator, will explain not only how to avoid readmission, but also how to use Oak Hill Hospital’s free Care Assure assistance program to do so. The Care Assure program utilizes Patient Care Navigators to coordinate post-acute care transitions for patients with cardiovascular disease and to ensure their follow-up care.
The presentation is not only open to patients but also to loved ones and caretakers who are invited to learn how to support this effort and improve their loved one’s overall health and wellness.
